26-27 K-12 Student Registration Packet
Registration for all new students occurs by appointment at the District Office, which is located at the north end of the high school at 6200 Ontario Center Road, Ontario Center, NY. Please park in the north lot on the left side of the entry driveway. The district office is directly across from this parking lot.
To schedule an appointment to register your child, please call the District Office at 315-524-1011.
Forms for registration or re-enrollment are available to pick up during office hours (8 a.m. to 4 p.m.) at the District Office or can be downloaded and printed out by clicking on the Enrollment Packet link below.
To use the fillable PDF Enrollment Packet, click the Enrollment Packet link below. Download the PDF to your computer. Open the document and type your information in the fillable fields. Save the completed PDF to your computer to print or email. Signatures will be required at registration.
Census Update
If you have children who are not school-age yet but will attend WCSD in the future, please register with the District now. This will be helpful as we plan for future years. Families can visit the pre-registration website to register their name and add their child to the District census.

Residency
To register your child, you must be a resident of the Fairport Central School District. Proof of residency is required when you register. Examples of proof of residency include a mortgage statement, signed purchase offer, or lease agreement.
Birth Certicates
Official documentation, such as a birth certificate, of age is required for all students.
Records of Immunization
Each child’s complete immunization record must be submitted from your physician or school at the time of registration. Immunization Documentation will be reviewed to confirm that vaccinations meet the appropriate number of doses and dosing intervals as required by New York State Department of Health. Failure to submit immunization documentation will result in exclusion of the child from school.- Polio – 3-5 doses * Diphtheria/Pertussis/Tetanus – 3-5 doses * Hepatitis B – 3 doses
- Pertussis – booster required for any child enrolled in 6th-12th grade —
- tetanus/diphtheria/pertussis (Tdap) vaccine
- Measles/Mumps/Rubella – 1 dose at age 12 months or older;
- 2nd dose of measles (rubella) vaccination (preferably MMR) after age 4
- Varicella – 2 doses
- Meningococcal (MCV4) – 1 dose (grades 7-11) and 2 doses (grade 12)
* Number of doses required depends on the age of the child when dose was given and grade student entering. A grace period may be applied for qualifying students. For information about medical exemptions, contact the School Nurse. For immunization assistance, contact the Wayne County Department of Health Immunization Program at (315) 946-5749.
Academic Records
Please register as early as possible so that screenings and/or guidance appointments can be scheduled. Copies of report cards, state testing scores, schedules, and transcripts are needed for placement. If your child has additional services, please bring a copy of his/her IEP or 504 plan.
Physical exam for your child(ren) required
All students new to the District must have a physical exam within the last 12 months. If your child has not had a physical, you may obtain the Health Appraisal form at the District Pupil Registration Office. The physical does not have to be completed by the time of registration; however, please contact the nurse at the school your child will be attending within two weeks to inform her of the physical date.
Dental exam for your child(ren) requested
All students new to the District are requested to submit a Dental Certificate based on a dental assessment within the last 12 months.
Homeless child(ren)
If your child is homeless, please contact the office of our homeless liaison at (315) 524-1004. -
- proof of custody or guardianship
- an affidavit (written statement signed under oath) saying that you have "total and permanent custody and control" over the child
- other proof such as documentation that the child has been placed with a sponsor by a federal agency.

- Lease or deed
- Affidavit from the person you pay rent to, stating that you live there.
- A letter from the person you pay rent to, stating you live there.
- Owner Statement of Residency - A notarized document stating you live with another person who resides within the district.
Other documentation, such as:
- Pay stub showing your address
- Utility bill or another bill in your name
- Membership documents based on residency, such as a local library card
- Voter registration card
- Driver's license, or permit, or non-driver ID
- State or other government-issued ID
- Documents from government agencies such as social service agency or the federal Office of Refugee Resettlement
- Custody or guardianship papers

- Certified Birth Certificate (from any country)
- Baptismal record (from any country)
- A passport (from any country)
If you don't have any of the above documents for proof of age you can use the following documents if you've had them for at least two years:
- Driver's license
- State or governmental ID
- School photo ID with date of birth
- Consulate ID card
- Hospital or health records
- Military dependent ID card
- Other documents from federal/state/local agencies (examples: Department of Social Services, Office of Refugee Resettlement)
- Court orders
- Native American tribal document
- Records from international aid agencies or voluntary agencies

UPK
2026-2027 SCHOOL YEAR UPK PROGRAM
In December 2025, the Wayne Central School District approved the adoption of a full-day Universal Prekindergarten (UPK) program to be housed at the Primary School. Full-day UPK will now be offered in addition to half-day UPK options provided by partnering community agencies.
Please Note:
- The full-day program will be at Wayne Primary School. There will be three classrooms.
- Hours will be 8:30 a.m.-2:00 p.m.
- Students will have lunch in school (bringing or buying).
- Curriculum will align with the NYS UPK Standards.
- Families will be responsible for transportation to and from the program.
Wayne Central School District welcomes all applicants for the UPK program. Slots will be limited, and selection will be implemented through a lottery system. We will have both full-day and half-day options available. Half-day UPK will be 2 ½ hours, with AM and PM options at our partnering community agencies.
Timeline:
- UPK registration packets will be sent out in early March 2026.
- Packets will have a due date to be returned in early April 2026.
- The UPK lottery will take place in the middle of April 2026.
- Results of the lottery will be communicated shortly after the selection.
- Students who are not selected, or who do not get their preferred choice, will be placed on a waiting list.

Universal Prekindergarten is a New York State funded program specially designed for 4-year-olds. In order to qualify for the 2026-2027 school year, children must turn four on or before December 1, 2026. In addition, children must reside in the Wayne Central School District. The UPK program is free to district residents, however, transportation will not be provided. The UPK program follows the same calendar as the school district calendar.

We are happy to announce that we will offer full-day UPK at Wayne for the first time in 2026-2027! The program will be housed in the Wayne Primary School and will operate 8:30-2:00, Monday-Friday. Our students will work with the Three Cheers for Pre-Kindergarten Curriculum. This resource aligns with NYS UPK standards and focuses on the following areas:
- Play-based learning
- Social emotional learning
- Language and communication
- Emergent Literacy - Reading
- Emergent Literacy - Writing
- Math
- Science
- Social Studies
- Arts
- Physical Development
Students will attend the program for 5 ½ hours a day, including lunch. The students will have access to our school facilities and our UPK teachers will work closely with our kindergarten teachers to prepare our youngest learners for their school journey.
We are excited to welcome three classrooms of UPK students to the Primary School. We look forward to partnering with families to provide the best possible education for each and every child.

Early Childhood is one of the most important stages in a child’s development. The attention we give our children at this early stage is crucial to their future success in school and life.
The purpose of Hop, Skip and Jump Preschool program is to plan and implement prekindergarten experiences that meet the developmental needs of the children and their families.
The Hop, Skip and Jump Preschool Program is based on the belief that all children can learn when given sufficient time for individual growth in a positive atmosphere. Our school experiences enrich children’s lives and provide them with skills necessary for lifelong learning.
Early exposure to formal schooling develops cognitive and social skills and respect for cultural and racial differences among children.
Our Program Offers:
- Opportunities for parents to become actively involved in their children’s education, an important aspect of early childhood development.
- Emphasis on developing positive self-esteem and high expectations for all students
- A focus on the individual learning styles of the children
- Oral and written language skills in all academic areas

KINDERGARTEN REGISTRATION INFORMATION
MESSAGE TO PARENTS OF ELIGIBLE KINDERGARTEN CHILDREN:
Children who turn five on or before December 1, 2026, are eligible for Kindergarten for the 2026-2027 school year.
o Kindergarten Registration Packets will be mailed out on March 6, 2026, to children who appeared in the district census report.
If you have a Kindergarten-eligible child and do not receive a registration packet in the mail, please complete the Digital Family Census Data Form.
o Registration appointments will begin on March 9, 2026.
o Please call 315-524-1011 to schedule a registration appointment.
